This Addendum attaches to: Personal Guarantee Addendum | Authority Lease Agreement | Dispatch Agreement | ACH Authorization Agreement

IMPORTANT: This Guarantor Identity Verification Addendum is a legally binding document. By signing below, the Guarantor certifies the accuracy of their identity information and agrees to provide documentation as required.

1 PURPOSE

This Guarantor Identity Verification Addendum ("Addendum") is entered into by the undersigned individual ("Guarantor") in connection with services provided by We Have Lots Of Loads LLC ("WHLOL").

  • This Addendum supplements the Personal Guarantee and any related service agreements
  • The purpose is to verify the identity of the Guarantor and reduce fraud, misrepresentation, and payment disputes

2 IDENTITY DISCLOSURE REQUIREMENTS

Guarantor agrees to provide accurate and complete identifying information, including:

Full Legal Name
Date of Birth
Residential Address
Government-Issued ID Number
State of Issuance
Last 4 of SSN (if required)

Guarantor certifies that all information provided is truthful and accurate.

3 DOCUMENTATION REQUIREMENTS

Guarantor agrees to provide:

  • A clear copy of a valid government-issued photo ID
  • A recent utility bill or document confirming residential address (if requested)
  • A voided check or bank letter confirming ACH account ownership (if applicable)

Failure to provide documentation may delay activation of services.

4 FRAUD PREVENTION

Guarantor acknowledges that:

  • Providing false identity information constitutes fraud
  • Fraudulent misrepresentation is grounds for immediate termination
  • Fraud may result in civil and/or criminal action

WHLOL reserves the right to verify identity information through lawful third-party verification services.

5 USE OF INFORMATION

Identity information will be used solely for:

Contract enforcement
Fraud prevention
Payment verification
Collections enforcement
Regulatory compliance
  • Information will be handled in accordance with WHLOL Privacy Policy
  • WHLOL does not sell personal identification data

6 ENFORCEMENT RIGHTS

In the event of:

Non-payment
Chargebacks
Breach of contract
Fraud
Back-solicitation violations

WHLOL may use verified identity information to pursue lawful remedies including:

  • Arbitration
  • Legal action
  • Collections
  • Credit reporting (where permitted by law)

7 CONTINUING CERTIFICATION

Guarantor certifies that:

  • They are the true owner or authorized signatory of the contracting entity
  • They have authority to bind the company
  • They understand they are personally liable under the Personal Guarantee

This Addendum survives termination of any underlying agreement.

8 GOVERNING LAW

This Addendum shall be governed by the laws of the State in which WHLOL is registered, without regard to conflict of law provisions.
